What to include

A measurement looks wrong
Tell us your device, browser, and what you measured against what. Include whether you had calibrated and what DPI the chip showed, since that one number usually explains it.
Something is broken
Which page, which browser and device, and what you were doing when it happened. A screenshot helps more than anything else.
A tool you'd like to see
If you looked for a measuring tool and could not find a decent one, that is exactly what we want to hear about.

Before you write

If a measurement is off, two things fix it almost every time: reset your browser zoom to 100% (Ctrl+0, or Cmd+0 on a Mac), then recalibrate with a bank card. Zoom silently rescales CSS pixels, and a calibration made on a different monitor will not apply to this one. The ruler and screen size checker both show your current calibration in the status chip, so you can see at a glance whether it is set.
